After completing the Red House puzzle, The Medium players will find themselves in the basement where they will encounter a mysterious door that appears to lead into a bunker. Unfortunately, to open it, players must first enter a four-digit code.

The solution to breaking the code to the bunker door falls under the password trope of birthdays and wedding anniversaries. To find the puzzle solution, players must first enter the side room directly across from the bunker entrance.

In this room, players will find a locker that is chained shut, and a paper receipt on the desk next to it. To open the locker, simply interact with it and select the bolt cutters that were obtained near the start of The Medium's campaign.

Inside the locker, players will find an empty box for a timepiece that was a 10th-anniversary present. If the player inspects the piece of paper on the desk next to the locker, they will find it is a receipt for the gift and is dated April 14th, 1976.

Now that the player knows the date the gift was purchased, we can discern that Thomas was married in 1966, which is the code to the bunker door. However, before players progress through the bunker door there are several missable collectibles that the player should pick up before proceeding.

In the basement, there are three missable "troubled man notes," and three echoes that players can find. While the troubled man notes are immediately added to the player's inventory upon inspecting them, to collect the echo players must interact with the object and activate their insight vision until an audio cue begins playing.

Players can find the first troubled man note directly across from the base of the stairs on a shelf and an additional one in the wine cellar room adjacent to it. The third "troubled man note" can be found in the main basement room, towards the back windows, underneath a stool.

The first echo can be found on a shoe directly outside the wine cellar room. The second echo can be found on a teddy bear in the main room towards the windows. And the final echo can be found on a syringe directly underneath the left window.

With all missable items in the basement collected, players can now unlock the bunker door and proceed to the game's most difficult puzzle located in the water pump room before facing off against the Maw.
